The procedures listed below should be followed when placing all orders. You have four options to choose from when placing an order: Phone, Fax, Mail-in, and E-mail.

Phone Orders:

When placing your order with our Supply Service Coordinator, it is important that you give your Company/Business name and provide a purchase order number that will identify your order for your records and ours. After this information has been recorded, the coordinator will ask you to give them the item number of material(s) you are ordering, the title(s) to verify that the correct item is being ordered, and the quantity that you are ordering.

Fax Orders:

The fax number that has been set up to receive your written order is: 217-787-5916.

When faxing your order to our supply coordinator please print off the form at the bottom of the page and completely fill it out and fax it to our office.

Mail-in Orders:

To place your order via mail, please print the Supplies Order Form, fill out completely and mail it to the following address:

Important Reminders About Ordering

Payments – Due 30 days after receiving shipment of materials.

Service Charge – The cost of shipping (UPS or First Class) is added to all orders.

Free Materials Policy – The American Lung Association offers the first $20 of materials free, one time a year. All other orders will be charged the price listed in the attached catalog.

Prices – All prices are effective January 2004 and are subject to change.

Discounts – Discounts are available if you order 1,000 or more of any item. Please call the Supply Service Center to verify discount.

Customer Service

Updates – Updates will be available through the on-line catalog.

Order Inquiries – If you have a question regarding a previous order or an invoice, or need to know the status of an order being processed, call 1-800-596-4872 or 217-787-5864.

Return Policy – All orders are returnable up to 30 days. During the 30-day period, from the date of purchase, you will be allowed to return your order for credit. Product held after 30 days is nonrefundable.
